Treatment of scar contracture of the hand and finger in children at the Saint Paul General Hospital

Abstract

This study is to evaluate the treatment results of scar contracture of the fingers in children under 6ys. This study describes a case series performed on 17 patients with 36 scars contracture of hand and finger at Saint Paul Hospital from 2020 to 2022.
The local flap was performed on 22/36 fingers, FTSG (full thickness skin graft) was performed on 11/36 fingers, and combined local flap and FTSG on 3/36 fingers. In the linear band scar-contracture group, 17/18 fingers were undergone the local flap. In the broad-band contracture group, 13/18 fingers required FTSG. At discharge time, all local flaps and skin grafts were alive, 1/36 finger was infected and completely healed in 2 weeks without surgery. The total active range of motion (TAM angle) increased by 75.2 degrees (98% of the deficit). 3 months after surgery, the average increase in TAM angle was 69.7 degrees (90%). Small flat scars appeared in 34/36 fingers, 2 fingers have hypertrophic and contracture scars. The study showed that the local flap is useful with linear band type, and FTSG should be an indication for broadband type. The results after 3 months of the TAM angle of the skin graft group improved less than the group using local flaps.
