Abstract

itation (CPR), resuscitation outcomes, and factors associated with the return of spontaneous circulation (ROSC) in patients with cardiac arrest treated using an automated chest compression device at the Emergency Department, Military Hospital 175.
Methods: This prospective single-arm study included 63 patients with cardiac arrest who were treated at the Emergency Department, Military Hospital 175, between June 2024 and December 2025. Circulatory parameters were measured serially in the same patients during the manual chest compression phase and the automated chest compression phase. Manual chest compressions were performed for the first 10 minutes in accordance with the 2020 American Heart Association (AHA) guidelines. Patients who did not achieve ROSC were subsequently switched to the LUCAS® 3 automated chest compression device, with study parameters during mechanical chest compression recorded up to minute 10.
Results: Among the 63 patients, the mean age was 58.2 ± 16.5 years, 69.8% were male, and 68.3% experienced out-of-hospital cardiac arrest. During the mechanical chest compression phase, the rate of femoral pulse detection and EtCO₂ and SpO₂ values were significantly higher than during the manual chest compression phase (p < 0.001). Patients who achieved ROSC had significantly higher EtCO₂ at minute 10 of mechanical chest compression than those without ROSC (22.1 ± 6.9 vs. 11.8 ± 5.6 mmHg; p < 0.001). An EtCO₂ ≥15 mmHg at minute 10 of mechanical chest compression was significantly associated with ROSC (OR = 15.41, 95% CI: 4.61–51.49; p < 0.001). The rates of ROSC, survival to hospital admission, survival to hospital discharge, and favorable neurological outcome were 42.9%, 28.6%, 12.7%, and 7.9%, respectively.
Conclusion: The use of an automated chest compression device was associated with increased circulatory parameters during cardiopulmonary resuscitation, as reflected by a higher rate of femoral pulse detection and improved EtCO₂ and SpO₂ values. EtCO₂ at minute 10 of mechanical chest compression was associated with the likelihood of ROSC.
