Abstract

Background: When restoring the appearance and function of the fingers, hand surgeons face a challenge in choosing a suitable surgical method to repair finger skin defects.
Methods: In this study, we designed a long elliptical flap based on a propeller perforator flap (PPF) and located slightly toward the dorsal lateral aspect of the finger. The flap with a pedicle consisting of the dorsal perforator of the distal digital artery and the dorsal digital artery perforator chain is rotated to cover a wound on the distal end. From January 2020 to May 2021, 34 patients with 37 fingers soft tissue defects were treated with the propeller perforator flap.
Results: All flaps survived after surgery, and 14 had transient venous congestion. After an average follow-up time of 7.89 months, the static two-point discrimination of the flap was 8.19 ± 1.97 mm, and the range of motion was 231.60 ± 23.20. All patients were satisfied with the results of the treatment.
Conclusions: PPFs are a reliable option for digital reconstruction, allowing surgeons to cover a variety of finger defects without sacrificing local vasculature; well flap survival and patient satisfaction.
