Clinical characteristics, ultrasound And magnetic resonance imaging findings in patients With ruptured silicone breast implants

Abstract
Objective: To describe the clinical characteristics and imaging findings on ultrasound and magnetic resonance imaging (MRI) in patients with breast implant rupture.
Methods: This descriptive study included 20 patients with suspected breast implant rupture who underwent surgical implant removal, with the diagnosis confirmed intraoperatively at the Department of Plastic & Aesthetic Surgery, Hospital of Post and Telecommunications, and the Department of Microsurgery, Maxillofacial Aesthetic and Plastic Surgery, and Laser, Hanoi Medical University Hospital between July 2024 and June 2026.
Results: A total of 20 patients (mean age: 44.3 ± 10.17 years) with 29 ruptured breast implants (mean implant age: 11.67 ± 3.9 years) were included. Among the ruptured implants, 28 were intracapsular and one was extracapsular. Seventy percent of the patients reported regular use of underwire bras, whereas none had a history of chest trauma. Most breasts (68.97%) were asymptomatic; the remaining breasts presented with breast tightness or pain. The majority of suspected ruptured implants showed no objective clinical signs and were classified as Baker grade I capsular contracture. Ultrasound demonstrated a sensitivity of 93.1% for diagnosing breast implant rupture. Among the diagnostic signs, the finding with the highest sensitivity was a hyperechoic periprosthetic fluid collection (100%). MRI achieved a sensitivity of 100% for the diagnosis of implant rupture, with the linguine sign being the most sensitive imaging finding.
Conclusions: Breast implant rupture is frequently clinically occult, with the absence of obvious subjective symptoms or objective physical findings. When present, clinical manifestations may include breast tightness, breast pain, and breast asymmetry. MRI demonstrated a sensitivity of 100% for detecting breast implant rupture, outperforming ultrasound, which showed a sensitivity of 93.1%.
